Comment: Routinely used in pregnancy screen for congenital open neural- tube defects in the fetus. Confirms significantly raised values with measurement of AFP in amniotic fluid. A low level may be found when a chromosomal abnormality is present in the fetus

High maternal levels (> 2 times the median) should be investigated by ultra sound examination to rule out incorrectly estimated gestation age, multiple gestation, fetal death, or obvious malformation e.g. anencephaly
Increased AFP concentration in maternal serum is associated with open neural –tube defect (NTD), omphalocele, esophageal or duodenal atresia and other conditions
Decreased AFP is indicated in Down’s syndrome, fetal demise, molar pregnancy, spontaneous abortion, trisomy 18, and overestimated gestational age
